Participation Agreement

Last updated in September, 2026

This Participation Agreement governs general access to and use of the Whole Person Longevity Program, including the Dailyhuman guided-reflection experience and the Viit H measurement station, when made available through an approved site, employer, institution, foundation, hospital, or community program.

This Participation Agreement is not a research informed consent form. If you are invited to participate in a formal research study connected to the Whole Person Longevity Program, the responsible research team will separately provide the study-specific informed consent form and any required privacy or authorization documents before study-related procedures begin. That informed consent form, not this Participation Agreement, governs your participation in the research study, including study procedures, risks, benefits, compensation, data handling, and withdrawal rights.

This document is a working template. It should be reviewed and approved by the applicable legal, privacy, compliance, and research-governance teams before publication or use.

1. Acceptance of this Agreement

Before participating in the Whole Person Longevity Program, you will be asked to review this Participation Agreement and indicate whether you agree to it. If you select ‘I Agree,’ you may proceed with the Program. If you do not agree, you should not participate in the Program.

If your participation is connected to an employer, health program, community site, or research study, additional notices, consent forms, privacy documents, or site rules may also apply.

2. Description of the Service

The Service is designed to support wellness, prevention, whole-person health, and behavior change through personal reflection, behavioral pattern identification, goal setting, and physiological feedback. The Service may include:

  • Dailyhuman / Whole Person Longevity Lab: an AI-powered experience that uses guided reflection and structured prompts to help users better understand behaviors, habits, thought patterns, barriers, and goals related to whole-person and cardiometabolic health.

  • Viit Health Station: a non-invasive, light-based health monitoring station that estimates physiological indicators such as oxygen saturation (SpO2), heart rate, blood pressure, and glucose or metabolic-related measures, where available.

  • Health and habits questionnaires: surveys or modules related to lifestyle, nutrition, movement, sleep, stress, nicotine exposure, whole-person health, and other wellness or prevention topics.

  • Personalized feedback: feedback screens, reminders, insights, or Mindset Cards that may summarize patterns, stages of change, goals, and reframed reflections based on your responses and program activity.

Available features may vary by site, country, institution, employer, study arm, or program configuration.

3. Wellness and prevention purpose

The Service is offered for wellness, prevention, education, engagement, and research-support purposes. It is intended to help users better understand personal patterns and support healthier habits over time.

The Service does not provide diagnosis, treatment, emergency care, or individualized medical advice. Information provided by the Service should not be used as the sole basis for medical decisions or changes to medication, diet, exercise, or treatment.

4. Eligibility and account registration

Eligibility may depend on the site or program through which you access the Service. Unless a specific site or research protocol states otherwise, you must be at least 18 years old to use the Service. Some formal research studies may apply additional inclusion and exclusion criteria.

When creating an account or registering for the Service, you agree to provide accurate and current information, including requested contact information, age or date of birth, language preference, and other details needed to operate the Service.

You are responsible for keeping your access credentials confidential and for activity that occurs under your account. We may suspend or terminate access if information is false, incomplete, unauthorized, or if this Participation Agreement is violated.

5. Program experiences and study arms

Depending on the site, you may receive different Service experiences. For example, some users may receive access to guided reflections, personalized Mindset Cards, and periodic reminders. Other users may receive questionnaires, Viit Health Station access, health-promotion materials, or measurement-only features.

In formal research studies, assignment to a particular experience may occur through randomization or other research procedures approved by the relevant ethics committee or Institutional Review Board. Those procedures will be explained in the study-specific informed consent form.

6. User responsibilities

You agree to use the Service only for lawful personal wellness, prevention, education, or approved program purposes. You also agree to:

  • Provide accurate information about yourself, including relevant health history, medications, and wellness behaviors, when requested.

  • Follow instructions provided in the app, by the site, or by authorized staff when using the Viit Health Station or completing program activities.

  • Use only your own account and not impersonate another person or enter another person’s information without authorization.

  • Not disassemble, tamper with, damage, reverse engineer, or misuse any device, software, QR code, prompt, content, or system connected to the Service.

  • Not use the Service in a way that interferes with its operation, violates applicable law, or infringes the rights or privacy of others.

7. Measurements, feedback, and limitations

The Viit Health Station may provide estimates or wellness indicators related to cardiometabolic health, such as SpO2, heart rate, blood pressure, glucose or metabolic-related measures, or other program-specific values. These measurements may differ from clinical reference measurements and may be affected by user positioning, motion, lighting, device conditions, physiology, medications, skin characteristics, timing of food intake, and other factors.

If a measurement is unavailable, appears inaccurate, or falls outside an expected range, the Service or site staff may recommend repeating the measurement, using an alternative assessment tool, or consulting a qualified healthcare professional. In formal research settings, additional procedures may apply as described in the informed consent form.

Do not use the Service in a medical emergency. If you think you may have a medical emergency, contact emergency services or seek urgent medical care immediately.

8. Guided reflection and AI-generated content

The Dailyhuman component may use AI-supported prompts and responses to guide reflection, summarize patterns, and generate personalized feedback. Because the experience is responsive to user input, prompts, summaries, and Mindset Cards may vary among users and over time.

AI-generated content is intended to support self-reflection, awareness, motivation, and healthy habit formation. It is not therapy, medical advice, diagnosis, or a substitute for professional mental health or medical care. You may skip questions or stop using the reflection feature if you feel uncomfortable.

9. Notifications and communications

The Service may send communications related to registration, reminders, reflections, measurements, follow-up visits, program updates, or account administration. Depending on the site and your preferences, communications may be sent by email, SMS, WhatsApp, app notification, telephone, or other approved channels.

You are responsible for ensuring that your contact information remains current. Message and data rates may apply depending on your carrier and service plan.

10. Data collected

Depending on the site and the features you use, the Service may collect or process:

  • Account and contact information, such as name, email, phone number, date of birth or age, language preference, and program/site identifiers.

  • Health and wellness information you provide, such as medical conditions, medications, height, weight, BMI, lifestyle behaviors, goals, questionnaires, and whole-person health responses.

  • Viit Health Station measurements and related device-quality information, such as estimated SpO2, heart rate, blood pressure, glucose or metabolic-related indicators, timestamps, device identifiers, and signal-quality information.

  • Reflection data, such as written responses, voice recordings, transcripts, AI-generated summaries, Mindset Cards, engagement activity, completion status, and feedback.

  • Technical and usage information needed to operate and protect the Service, such as login activity, feature usage, browser or device information, and security logs.

Specific data elements may vary by site, country, program configuration, and whether you participate in a formal research study.

11. How data may be used

Data may be used to:

  • Operate, maintain, and personalize the Service.

  • Provide measurements, questionnaires, guided reflections, reminders, feedback, and support.

  • Help users track their own information and engagement over time.

  • Help authorized program operators understand participation, engagement, barriers, and behavioral themes at an individual or aggregated level, depending on the program design and applicable permissions.

  • Improve the safety, usability, reliability, quality, and performance of the Service.

  • Support formal research activities only when permitted by an applicable informed consent form, privacy authorization, ethics committee/IRB approval, or other required legal basis.

Data handling will be further described in the applicable Privacy Policy, site notice, research informed consent form, HIPAA authorization, or other privacy document, as applicable.

12. Data sharing

Data may be shared with authorized parties as needed to operate the Service, support the program, comply with law, or conduct approved research. These parties may include the program operator, the site or institution through which you access the Service, Viit Health, Dailyhuman, approved technology vendors, authorized research personnel, and academic or health institutions affiliated with the program.

Where feasible and appropriate, data may be coded, aggregated, or de-identified before sharing. In formal research studies, the study team may use a unique study identification number and maintain any linkage file separately with access limited to authorized personnel.

For some research uses, only limited or de-identified data may be shared, such as age in years, gender, BMI, chronic disease status, laboratory values, and other data elements described in the study-specific informed consent form. Your identity should not be associated with research data unless permitted by the applicable consent and privacy documents.

13. Data security and retention

Reasonable administrative, technical, and physical safeguards are used to protect data, which may include password-controlled access, encryption where appropriate, secure servers, access restrictions, audit controls, coded study identifiers, and secure storage systems.

In research settings, data may be stored in systems approved by the responsible institution, such as REDCap, Box Health, institutional records management systems, or the Whole Person Longevity Lab platform, according to the approved protocol and applicable law. Research records may be retained for the period required by the responsible institution, sponsor, ethics committee/IRB, or law.

No system can guarantee absolute security. You should use reasonable care when accessing your account and should not share access credentials with others.

14. Formal research studies

Some users may be invited to participate in research related to the Whole Person Longevity Program. A research study may include additional procedures such as screening, informed consent, questionnaires, anthropometric measurements, blood pressure readings, glucose testing, blood draws, interviews, audio recording, randomization, or follow-up visits.

Participation in research is voluntary and separate from accepting this Participation Agreement. Before study-related procedures begin, the responsible research team will provide the applicable informed consent form in the required language and format. The consent form will describe the study purpose, procedures, risks, benefits, compensation, data uses, confidentiality protections, withdrawal rights, and contact information.

If there is any conflict between this Participation Agreement and an approved research informed consent form for a specific study, the approved informed consent form controls for that research study.

15. Withdrawal, account deletion, and continued records

You may stop using the Service at any time. You may also request account deletion or data-access support using the contact information provided by the program operator, subject to applicable law, contractual obligations, institutional policies, and technical limitations.

If you are participating in a formal research study, withdrawal from the study and treatment of data already collected will be handled according to the study-specific informed consent form and IRB/ethics-approved protocol.

16. Intellectual property

The Service, including software, prompts, content, trademarks, designs, measurement systems, models, reports, user interfaces, and other materials of Dailyhuman, Viit Health, or their respective licensors, remains the property of its respective owner. Using the Service does not grant you ownership rights in the Service or permission to copy, modify, distribute, reverse engineer, or commercially exploit it.

17. Third-party services

The Service may use third-party infrastructure, communication tools, hosting services, analytics, secure storage, or other vendors to operate the program. These providers may process data as necessary to provide their services and are expected to follow applicable contractual, privacy, and security requirements.

18. Limitation of liability

To the extent permitted by applicable law, the program operator, Viit Health, Dailyhuman, affiliated institutions, and their respective personnel are not liable for indirect, incidental, special, consequential, or punitive damages arising from use of or inability to use the Service. The Service is provided on an "as is" and "as available" basis, without warranties except as required by applicable law.

Nothing in this Participation Agreement limits rights that cannot be limited under applicable consumer, health, privacy, research, or public-policy laws.

19. Changes to the Participation Agreement

We may update the Participation Agreement from time to time. Material changes will be communicated through the app, site, or contact method provided, where required. Continued use of the Service after notice of the updated Participation Agreement constitutes acceptance of the updated Participation Agreement, unless applicable law requires a different process.

20. Governing law and site-specific addenda

This Participation Agreement is governed by the laws of Delaware, without prejudice to mandatory legal rights that may apply in your place of residence or participation. Site-specific addenda may be used to address local laws, institutional policies, privacy requirements, language requirements, or research-governance obligations.
